What is GAPP?

GAPP is a medicaid program curated for medically qualified children from infant to 20 years old. The GAPP program was created to support families that have children who are medically fragile and are in need of skilled nursing services.

How to apply

  • Step 1. Meet with GAPP Provider

    Schedule a call or visit with a Proud Life Home Care coordinator, and we will help with the initial document process to get you started.

  • Step 2. Complete Consent Forms

    Sign a few documents that will help us create a service plan for the needs of your loved one and allow us to provide care.

  • Step 3. Process Documents

    Once all of the necessary documents are collected and reviewed, we then send all forms signed by you to Medicare for approval.

  • Step 4. Approval or Deniel Letters

    Medicaid will review all documents submitted and provide an approval or denial letter that will be provided to you within 30 to 60 days of the processing date.

  • Step 5. Let the care Begaine

    After approval, we will provide you with one of our assigned medical professionals, and services will begin.
